How to Participate
Pressure ulcer incidence and prevalence remain high and costly among nursing home residents. Every year, pressure ulcers affect more than 1 million acute and long-term care patients, and more than $355 million is spent annually on pressure ulcer treatment in long-term care settings. Treatment is estimated as 2.5 times that of prevention, therefore highlighting the importance and cost-effectiveness of a prevention program.
On-Time is a practical approach to embedding HIT into front-line practices to improve clinical outcomes in long-term care facilities. The On-Time program blends QI and IT by redesigning processes for earlier identification of high-risk residents and integrating user-friendly clinical decision support (CDS) tools into those processes
The On-Time Pressure Ulcer Prevention program standardizes CNA documentation data elements and facilitates the use of weekly, actionable reports to identify and track pressure ulcer risk factors by resident. These practices are integrated into everyday workflow, allowing staff to intervene in a timely manner with at-risk residents to reduce the incidence of pressure ulcers.
On-Time quality improvement process is currently embedded in health IT for 10 vendors and 80+ nursing homes across the country have adopted it.
